The Chicago Department of Public Health, in collaboration with the Mayor's Office, has committed more than $9 million to the Healthy Chicago Zones (HCZ), a 4.5 year initiative that addresses the racial disparities that prevent many Chicago residents from being healthy. Breaking the city into six regions, the HCZ collaborates with regional and community organizations to meet the particular needs of Chicago residents to increase their health and wellness outcomes. In the Southwest region, the Southwest Organizing Project is the regional lead.
Public Health Ambassadors, or PHAs, are community residents with a passion for public life and are the movers of the Healthy Southwest work. With eleven PHAs across the southwest, they work on the ground, outreaching in the community to connect residents to the information, resources, events, and opportunities they need to be healthy. Their work is more vital than ever as we work towards comprehensive wellness, while also building community and ensuring the voices of residents are heard. As of January 1 2023, our Public Health Ambassadors have made over 5000 referrals to local service providers, with over 80% resulting in connection or enrollment in services
